Jan La Belle Scholarship Application Now Available!
The application for the Jan La Belle Scholarship is now available on our website at www.fndfl.org/scholarship.htm. Applications are being accepted until November 22, 2010. Winner(s) will be notified by March 30, 2011.
Awards will be made annually, up to $15,000. They will be targeted at individuals between the ages of 16-24 who have a disability and who are pursuing higher education as a first-time student. Their school of choice may be academic or vocational.
The Jan La Belle Scholarship Program was created to honor Jan La Belle, Executive Director of Family Network on Disabilities from 1995 to 2005, for her passion and dedication to individuals with disabilities and their families.
We at FND are grateful for her service and wish to recognize her dedication by helping ensure that each person, regardless of ability or disability, is able to pursue their own hopes and dreams and to do so as independently as possible. This program is created to allow any qualified individual with a disability to better themselves and give back to the community.
UnitedHealthcare Children's Foundation
The UnitedHealthcare Children's Foundation (UHCCF) is a 501(c)(3) non-profit charity dedicated to improving access to medical-related services.
Families with children ages 16 and younger and who have a commercial health benefit plan can apply to UHCCF for grants up to $5,000; even those families not covered by UnitedHealthcare.
A grant from UnitedHealthcare Children's Foundation provides financial relief for families who have children with medical needs not covered or not fully covered by their commercial health benefit plan.
Who is eligible for a grant? The full list of eligibility criteria is available on www.uhccf.org. Here are a few of the criteria:
- Families who have a child or children with medical needs that are not covered or not fully covered by their commercial health benefit plan;
- Financial need of the child's family will be a consideration;
- The application must be submitted to the Foundation before the receipt of services. The Foundation does not pay for past medical expenses.

How does the grant work? If a grant for your child is approved, the grant will help pay for approved medical services/items after your commercial health benefit plans submits payment, if any. The grant funds are not paid to you or your child outright; you work with UHCCF to submit invoices/bills for approved medical services/medical items after your commercial health benefit plan submits initial payment (if any) to the health care provider.
What is the process for applying? For specific information about how to apply for a grant, please log onto www.uhccf.org. Once on the website:
- Click on the "To Apply" menu item, then click on "Applicant".
- Read through the information about the grants. Please be sure to look at the criteria and application checklist to see what information you may need to collect for the application. If you meet the criteria and have all the items listed in the checklist, then you can apply for a grant online.
- As you enter the application process online, you will be led through a pre-screening questionnaire to determine eligibility.
- If you pass the pre-screening questions, you will be offered the full online grant application.
- Please note that though the application is sent online, some required paperwork must be mailed to UHCCF.
How long does the application process take? The online application itself will take approximately 15-20 minutes to complete. The entire application review process varies. If you apply on behalf of your child, meet the stated criteria and have the items in the checklist on hand, please submit an online application.
FND's 25th Anniversary Tour
FND's 25th Anniversary Tour stopped in Gainesville on September 18th. This was number 7 of 13 planned celebrations that provide food, fun, and resources absolutely free to families across Florida. Stay tuned for additional information on the events being planned in Tampa, Ft. Lauderdale, Naples, Miami and Big Cypress.
"Teaming With Heart" Southern Regional Conference
Please join us on Saturday, September 25 at Sugarloaf School (Mile Marker 19, Sugarloaf Key) for the PEN (Parent Education Network) Southern Regional Conference, "Teaming With Heart". We will have a full day of workshops on topics ranging from Early Childhood through Transition. Workshops will be presented in English, Spanish, and Creole.
Special Guest: Roger Mills , Senior Attorney for the Office of Civil Rights will be presenting a full day on Section 504!

Pub Crawl Olympics Sponsored by Dunedin Waterside Rotary Club to Help Wipe Out Polio
Our friends at the Dunedin Waterside Rotary Club are hosting a pub crawl to benefit Rotary International's Global Campaign to wipe out polio.
Check out all of the information below and hopefully we'll see you there!
What: Put on your best jock or cheerleader outfit and show your team spirit. Teams of two to five people will crawl their way through downtown Dunedin testing their skills at Beer Pong, Flip Cup, Darts, Cornhole, and Beer Battleship. Compete for prizes to appear in the "Pub Crawl Olympics" Hall of Fame. Costumes optional but strongly encouraged. Benefits the Rotary International global fight against polio. Price includes a beverage at each of the 5 event locations. Must be 21 years of age to participate.
When: October 2, 2010. Registration begins at 6:00pm, the event starts at 7:00pm.
